Job overview

The Consultant Practitioner for Home Treatment Service leads the Home Treatment Service alongside the SAS Doctors West Kent.

We are a community team made up of a variety of staff including Specialist Doctors, Speciality Doctors, Advanced Clinical Practitioners and Health Care Assistants.

The Home Treatment Services have excellent links with Primary Care, Community Teams, Hospice, SECAMB Trust and Acute Trust Teams.

Main duties of the job

The post holder will be a clinical expert in Frailty and will fulfil the 4 domains of Advanced Clinical Practice:

1. Advanced and expert level clinical practice within the field of frailty ensuring clinical excellence
2. Leadership and consultancy providing expert professional advice to patients, carers and colleagues
3. Development and provision of education and training to other staff and trainees
4. Service development, evaluation, research and audit.

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Person specification

Qualifications

Essential criteria

1. First level Registration with NMC, HPCP or GPHC
2. Demonstratable, Significant or Extensive post registration experience
3. Teaching qualification
4. Research qualification
5. Non- medical Prescribing

Desirable criteria

6. Psychology or counselling experience and training/ education

Experience

Essential criteria

7. Extensive clinical experience in the care and treatment of people with frailty (including long term conditions and dementia care experience)
8. Advanced skills in history taking and physical assessment of frail people
9. Independent prescribing experience
10. Experience of advanced specialist interventions in the assessment and management of older patients
11. Experience of planning, implementation and delivery of educational programmes relating to long term conditions

Desirable criteria

12. Extensive experience of presenting report/ educating / information to public forums
13. IR(ME)R training or equivalent

Knowledge

Essential criteria

14. Advanced understanding of long-term conditions and application to older people
15. Advanced knowledge of clinical governance
16. Advanced knowledge of change management
17. Advanced knowledge of managing complex patient information and situations

18. Extensive research knowledge
