We are looking torecruit an enthusiastic and committed Practice Nurse with the desire todeliver a high quality of care and to continue the development of our wellestablished and well regarded practice nursing team.


Main duties of the job

Thepost holder is responsible for the delivery of general practice nursing to thewhole practice population. They will provide and maintain a high standard ofquality evidence based nursing care to meet the needs of patients, as well asproviding nursing assistance to the doctors and other members of the primaryhealthcare team. The duties will include all tasks normally undertaken by anexperienced registered practice nurse and in addition any roles agreed between the nurse and thedoctors as appropriate, in particular the assessment and management of patients with chronic diseases according to areasof knowledge and responsibility.

We are a large, well organised and friendly practice located in a purpose-builthealth centre in a semi-rural location in South Cheshire, with a patient list size of over 20,000 patients.

JOB DESCRIPTION &PERSONAL SPECIFICATION

JOB TITLE: PracticeNurse

SALARY: Dependent on experience

APPOINTMENT: PermanentContract

REPORTS TO: Partners and Lead Nurse(Clinically), Lead Nurse and Practice Manager (Administratively)

JOB SUMMARY

Thepost holder is responsible for the delivery of general practice nursing to thewhole practice population. They will provide and maintain a high standard ofquality evidence based nursing care to meet the needs of patients, as well asproviding nursing assistance to the doctors and other members of the primaryhealthcare team. The duties will include all tasks normally undertaken by anexperienced RGN within a primary care setting and in addition any roles agreedbetween the nurse and the doctors as appropriate, having regard to currenttraining. The post holder will use their own judgement, resourcefulness andcommon sense and show flexibility with regard to rotas and working patterns.

DUTIES ANDRESPONSIBILITIES

Clinical Skills:

Undertakeassessment and management of patients with chronic diseases according to areasof knowledge and responsibility.

Offeradvice about childhood and adult vaccinations and ensure vaccines areadministered under patient group directions.

Runningwell person clinics and health promotion programmes by providing a holisticassessment and lifestyle advice on diet, smoking, alcohol intake and exercise.

Assessingproblems presented opportunistically by patients, dealing with minor illness.

Performa holistic assessment of patients attending for cervical cytology smear tests.

Providea holistic approach to wound management and implement wound care in line withcurrent evidence based guidelines. Suture removal, Wound care / ulcer care

Requestingpathology tests for example urine culture, swabs

Offera holistic approach to travel health by providing comprehensive advice topatients prior to travel including; vaccinations and medicines, safe sex, foodhygiene, sun protection.

Ableto document Consultations according to NMC guidelines, at all times there willbe a need to maintain accurate records.

Obtainand document informed consent, (either verbal or written).

Awarenessof statutory and local clinical protection procedures, including systems ofreferral.

Abilityto recognise signs and symptoms of child abuse.

Ableto recognise, manage anaphylaxis in accordance to the current UKguidelines.

Ableto perform Cardio-pulmonary resuscitation, in accordance to current UKguidelines.

Ensureinfection control guidelines are maintained.

Monitorand manage maintenance of stock (including vaccinations) and equipment toinclude refrigeration and emergency equipment.

Followagreed clinical protocols with referral to Senior Nurses or GPs as appropriate

Contributeto the assessment of service needs. Assist patients to identify their healthneeds. Contribute a nursing perspective to the Practice development plan.Manage and organise individual patient consultations. Aware of identificationand reporting procedures related to professional standards.

Other tasks will include:

Thenursing team are responsible for maintaining and cleaning equipment used by thenurses and GPs

Thenursing team are responsible maintaining GP and Nurses rooms, stocking androtating items as required Maintain general tidiness and cleanliness of nursesand treatment rooms

Chaperoningand assisting patients who are being examined by another clinician.

Participatein any Practice meetings when required.

Onoccasion there may be a requirement to undertake home visits

Anyother delegated duties appropriate to the post

Identifychanges to clinical practice that are required to implement evidence-basedguidelines.

Personal/Professional Development:

Thepost-holder will participate in any training programme offered by the Practiceas part of this employment.

Thepost holder will be required to undertake all mandatory training required forthe role.

Participationin an annual individual performance review, including taking responsibility formaintaining a record of own personal and/or professional development

Takingresponsibility for own development, identifying training needs and performanceand demonstrating skills and activities to others who are undertaking similarwork

Participatein continuing education and maintain a contemporary level of professionalknowledge and skills.

Participatein clinical supervision.

Supportothers with their training and development needs i.e. student nurses, peers

Attendingstaff meetings and practice team meetings.

They may also have access to information relating to thePractice as a business organisation. All such information from any source is tobe regarded as strictly confidential information relating to patients, carers,colleagues, other healthcare workers or the business of the Practice may onlybe divulged to authorised persons in accordance with the Practice policies andprocedures relating to confidentiality and the protection of personal andsensitive data

Travel

The nature of your employment requires youto travel within a 10 mile radius of this address, to meet the needs of thebusiness. Where you are required to use your own vehicle to carry out yourduties, you must ensure that your vehicle is insured accordingly for businessuse.

The role of thePractice Nurse is an evolving one and it is anticipated that the post holderwill be responsible and reasonably flexible in the interpretation of the abovejob description. This job descriptioncovers the main duties which the post holder will be expected toundertake. These duties may be revisedfrom time to time following consultation with the post holder.
