An exciting opportunity has arisen foran 8-month fixed term or secondment for a B5 Mental Health Practitioner theWorcestershire Neighbourhood Mental Health Service covering the Droitwichlocality.
hours during Monday to Friday.

This role allows the possibility ofhybrid working, meaning you will be required to commute to the stated base 2 or3 days per week but allowing you to based within any commutable distance forthe remainder of your week. We would need to discuss the suitability to fitthis around the service at interview.

Main duties of the job
Will work as akey member and resource of the MDT core team.
This post willoffer a challenging and exciting opportunity to gain experience of workingacross North Community Mental Health Service, developing relationships withPrimary Care, partner agencies and wider mental health services.
You will havethe opportunity to design and deliver evidence-based recovery focused mentalhealth interventions and groups. Knowledgeof delivery psychoeducation within a group environment and 1:1 would be anadvantage.
You will haveexperience of working with this user group in community settings.
You will hold a blended caseload of common mental health disorders, severe mentalillness patients, Primary and Secondary, including those with emotional needs.
The ability towork flexibly as the need of the service demands is essential.
Attend weekly MDT meetings, participatein professionals meetings, signpost to adjunct services depending on patientneed.
Support and guidance to the carers ofpatients accessing the services

Job responsibilities

Provide specific PsychiatricNursing interventions to allocated patients/clients and their relatives who arereferred to the service.
Work in conjunction with allprimary care services and partnership agencies to foster closemultidisciplinary and multi-agency working.
Work across all clinicallyappropriate patients, 17-years-old and over, to deliver evidence-basedinterventions.
Under the direction of ServiceLead and as part of the Community Mental Health Service, provide prompt localassessment, triage, risk assessment, risk management, signposting, onwardreferral and evidence-based interventions for people presenting with mentalhealth problems.
Assist with the teams patientclinics and medication monitoring/administration.
Represent theteam on specific projects and working groups as required.
