The appointee will form part of our Acute Frailty team providing rapid Comprehensive Geriatric Assessment to frail patients in the Emergency Department, as well as contributing to the care of frail inpatients on our Older Peoples Medicine Unit, and outpatient Frailty Assessment Clinics. Advert Older adults living with frailty form a high proportion of our medical inpatient caseload, and this is projected to grow further in future. We are currently developing a rapid, consultant led Comprehensive Geriatric Assesment pathway in the Emergency Department and Same Day Emergency Care. The appointee will form part of this team one day a week, working alongside allied health professionals, resident doctors and advanced care practitioners. They will also work alongside 2 other consultant geriatricians and an experienced Specialty Doctor to provide care on our recently refurbished Older Peoples Medicine unit. We are also increasing the provision of rapid assessment outpatient clinics as an alternative to admission for older people living with frailty. IMT trainees and Foundation doctors rotate through the Older Peoples Medicine unit along with Geriatrics SpRs. The appointee will be expected to contribute to teaching and training on the ward and frailty teaching throughout the hospital. Older Peoples Medicine · Regular ward rounds on the Older Peoples Medicine unit · Weekly rapid access Frailty Assessment Clinic · Delivering weekend ward rounds and frailty assessment on the Acute Medical Unit and Same Day Emergency Care unit on a 1 in 6 basis · Participation in the General Internal Medicine out of hours rota (weekdays only), currently 1 in 16 Acute Frailty · Consultant-led frailty reviews in the Emergency Department and Same Day Emergency Care on one day per week, working alongside the Early Intervention Team physiotherapists and occupational therapists. · Support development of expanding frailty pathways, frailty assessment units, and urgent frailty clinics.
