Social Worker – Adult Social Care Learning and Disabilities

We are seeking a committed and motivated Social Worker to join our Adult Social Care service within the People Care – Social Care and Education Professional Pathway. This is an excellent opportunity to make a meaningful difference by supporting adults with diverse and complex needs, using a strength-based and person-centred approach.
As a Social Worker in Adult Social Care, you will:
Undertake Care Act assessments and reviews for adults with varying levels of need
Work collaboratively with individuals, families, carers, and partner agencies
Develop outcome-focused support plans that promote independence and safety
Manage risk and complete safeguarding enquiries under Section 42 of the Care Act
Participate in duty and act as a trusted assessor.
Prepare and contribute evidence for Court of Protection applications with guidance from a Practice Supervisor.
Professional Development
Newly qualified Social Workers will complete the Assessed and Supported Year in Employment (ASYE) within their first 12 months
Opportunities for progression to Advanced Social Worker (Grade 11) through the Progression Pathway
Ongoing supervision and professional support
Essential Requirements
Degree or equivalent qualification in Social Work
Registration with Social Work England
Experience in adult social care, including assessments, reviews, and safeguarding
Strong communication, analytical, and report-writing skills
Knowledge of relevant legislation and strength-based practice
Ability to work independently and collaboratively
