Overview
Avonbourne ward is currently looking to recruit a proactive and patient centred Discharge Coordinator. As a ward we pride ourselves on our effective teamwork. As a discharge Coordinator on Avonbourne ward, you will be required to interact closely with all members of the Multi-Disciplinary Team and other external health and social care services in order to facilitate safe and effective discharges for our patients.
Base Location: Poole Hospital
