<p>Wirral Council is looking for a Care Navigator to join the West Wirral Adult Social Care Team. This role, requiring 36 hours per week, involves supporting adults within the community through assessments and support planning. Ideal candidates will have strong knowledge of the Care Act 2014 and relevant experience.</p><p>Applicants must have good IT skills, be able to work independently, and thrive in a fast-paced environment.
