Job Description The postholder will be responsible for taking a lead role in working with adults, families, service providers and social work teams to achieve Aberdeenshire's HSCP aims around outcomes focussed commissioning. The role will lead practice to ensure a consistent approach to supported living services' transition from ‘time and task’ model support to commissioning for outcomes and weekly budgets. Provide day-to-day support to a team of professionals and support staff by contributing to the assessment of complex needs and service requirements, managing service delivery and contributing directly to the development and improvement of service provision to vulnerable adults, their families and carers There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the Service and the organisation.
