The Patient Care Coordinator works with both patients and Healthcare Professionals to ensure seamless, organised and effective care.
This role involves working closely with patients and healthcare providers, to facilitate communication, manage appointments, and support patient care plans.
Additionally, this position will support learning and development initiatives while ensuring adherence to CQC regulations.
Main duties of the job
1. Patient Liaison: Serve as the primary point of contact for patients, addressing their queries and concerns, and ensuring they understand their care plans.
2. Care Coordination: Coordinate patient appointments, referrals, and follow-up care, ensuring timely access to services and continuity of care. Support healthcare initiatives such as breast screening, bowel screening, and vaccinations.
3. Communication: Facilitate effective communication between patients, healthcare providers, and external agencies to support integrated care delivery.
4. Record Management: Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with data protection regulations.
5. Support Services: Assist patients in accessing support services, such as social care, mental health services, and community resources. Link with our Social Prescriber.
6. Quality Improvement: Participate in quality improvement initiatives to enhance patient care and service delivery.
7. Team Collaboration: Work collaboratively with healthcare teams to support patient-centred care and contribute to multidisciplinary meetings.
8. Reception: Help out on reception when needed, taking calls from patients and making appointments.
9. Coordinate the Patient Participation Group: Manage and support activities related to the patient participation group.

Qualifications:
* Education: Educated to GCSE level.
* Experience: Previous experience in a healthcare or care coordination role is preferred.
* Skills: Strong organizational, communication, and interpersonal skills. Ability to work independently and as part of a team.
* Attention to Detail: Ability to manage multiple tasks and maintain accuracy in a fast-paced environment.
* Problem-Solving: Strong problem-solving skills to address patient needs and coordinate care effectively.
* Empathy and Compassion: Demonstrated ability to provide empathetic and compassionate support to patients and their families.
* IT Proficiency: Proficient in using healthcare information systems and standard office software. Full support and training will be provided.
