Job Summary The post holder will be responsible for accurately extracting and translating clinical information from patient records into coded data using ICD-10 and OPCS-4 classifications, in accordance with National Clinical Coding Standards. This coded information plays a vital role in supporting the HealthHarmonie and Trust, clinical governance, audit, commissioning and performance monitoring. As an Accredited Clinical Coder, you will code inpatient episodes independently, including complex cases, ensuring accurate and timely submissions in line with HealthHarmonie and Trust's targets. You will also liaise with clinical staff to clarify documentation where necessary. Key Responsibilities Clinical Coding Duties Extract, interpret, and code relevant clinical data from multiple sources, including: Medical case notes Electronic discharge summaries Diagnostic reports (radiology, pathology) Electronic Patient Records (EPR) Assign diagnosis and procedure codes using ICD-10 and OPCS-4 in line with current national coding standards and HealthHarmonie and Trust guidelines. Accurately code complex and multi-specialty inpatient episodes, including long-stay patients and high-cost drug cases. Identify and flag documentation issues or ambiguous clinical content to the clinicians as appropriate. Ensure coding output supports PbR, SUS reporting, clinical audit, and national data requirements. Data Quality and Performance Achieve and maintain the required accuracy and productivity targets as determined by the Trust and external audit requirements. Conduct data validation and quality assurance checks on own coding output. Participate in clinical coding audits and address any coding discrepancies identified through internal or external reviews. Maintain up-to-date knowledge of changes to coding classifications, standards, grouping logic, and tariff implications. Team Contribution and Professional Development Participate in team meetings, CPD activities, and internal training sessions. Maintain NCCQ/ACC status through annual CPD and participation in professional development opportunities. Person Specification Essential National Clinical Coding Qualification (UK) - Accredited Clinical Coder (ACC) Proven NHS coding experience of inpatient episodes in an acute hospital setting In-depth knowledge of ICD-10 and OPCS-4 classifications and national coding standards Good understanding of medical terminology, anatomy, and disease processes Strong analytical and interpretation skills, with the ability to handle complex clinical data Ability to work independently, meet deadlines, and manage competing priorities Excellent attention to detail and commitment to maintaining high levels of coding accuracy and data confidentiality Competence in using EPR systems and clinical coding software Desirable Familiarity with grouper logic, HRGs, and the implications of clinical coding on Trust income Experience with coding software packages (e.g., 3M, Medicode) and Trust clinical systems Previous involvement in peer review, audits, or mentoring within a coding team Interest in future development toward Clinical Coding Auditor or Trainer roles Additional Information Hybrid working arrangements may be considered, subject to operational needs and ongoing performance The post holder is expected to uphold HealthHarmonie and the Trust's values and contribute to a culture of high-quality, patient-focused service delivery. Occasional travel between sites may be required for training, audits, or meetings.
