Overview
Staff Nurse (Adult Community Services - Day Hospice & Hospice at Home) at The Shakespeare Hospice, covering South Warwickshire. Part-time role with daytime shifts (8.30am–4.30pm or 2.00pm–10.00pm) over a 7-day rolling week; some evenings but no overnight shifts. This role is part of the Hospice at Home and Day Hospice teams, providing holistic, person-centred palliative care to patients and support to families and carers.
Responsibilities
* Act as a role model and provide exceptional nursing care across The Shakespeare Hospice Adult Community Services (Hospice at Home and Day Hospice).
* Provide physical, psychological, social and spiritual care for patients and carers.
* Holistically assess, plan, implement and evaluate patient care; review patient-led goals and outcomes through care planning.
* Undertake specialist patient-centred assessments to support end-of-life care or to maximise independence and quality of life.
* Identify and respond to changes in the patient’s condition and symptoms; adjust care with support from Team Leaders.
* Respond to patient crises/deterioration with an effective plan of care.
* Ensure infection control precautions are consistently observed.
* Liaise with other professionals and organisations to facilitate appropriate patient care.
* Participate in advanced care planning discussions with patients and carers, respecting their priorities and preferences.
* Maintain confidentiality and adhere to local and national policies/legislation.
* Advocate for patients to enable informed decision making; support carers and bereaved carers with their psychosocial needs.
* Use NICE guidelines and national standards to underpin evidence-based practice; communicate effectively in sensitive conversations.
* Complete contemporaneous, accurate clinical records; manage caseload from referral to discharge; prioritise and adapt workload as needed.
* Work autonomously and with the Hospice team; participate in MDT meetings, handovers and case conferences.
* Contribute to health promotion and educational programmes for patients, families and carers; be a source of expertise in hospice palliative care.
Managerial/Organisational
* Supervise and support other team members and volunteers; act as a role model upholding hospice values.
* Support junior staff with 1:1s, appraisals and objective setting; deputise for Team Leaders when required.
* Demonstrate proficient IT skills for data collection and reporting; assist with local audit and improvement plans.
* Contribute to service development; address risks and incidents per hospice policy.
* Ensure all clinical staff and volunteers are equipped and trained; adhere to policies and the NMC Code of Conduct and revalidation requirements.
* Engage in appraisal processes and clinical supervision; participate in induction and learning opportunities.
* Promote the charity and its services at events where clinical support is required.
Educational/Development
* Complete statutory and mandatory training; maintain ongoing CPD; stay informed on policy and developments in palliative care.
* Be prepared to teach and share knowledge with the wider teams; support nursing and medical students and work experience students.
* Participate in induction programmes for new staff and volunteers.
Qualifications
* Essential: Degree or Diploma in Adult Nursing (RGN); current NMC registration; evidence of ongoing CPD.
* Desirable: Further post-qualification training (e.g., oncology, palliative care, bereavement support); mentoring/assessing qualification.
Experience
* Essential: Previous experience in palliative/end-of-life care; ability to organise care programmes; experience across community settings; MDT working; autonomous and team working; supervision of staff/volunteers; risk management; caseload management; safeguarding; IT skills; teaching and communication skills; motivation and compassion; willingness to support service development; current driving license for business use.
* Desirable: Two years post-qualification experience; community experience; experience in advanced care planning; experience as a practice assessor; bereavement support interest.
