Hourly Rate: £13.50 p/h
Job Purpose:
The role includes working within an allocated zone, responsible for evaluating and assessing new client packages, developing care plans, carrying out risk assessments, conducting reviews of existing packages, liaising with Commissioners and clients, evaluating care progress and recommending changes to the care plan if required. Mentoring and leading a team of Care Support Workers. Carrying out staff observations, and supervisions.
You will also be part of the on-call rota on a rotational basis.
A full driving licence and access to own transport is essentials essential.
Role Responsibilities:
As the role can change on a daily basis, you will need to be highly organised, efficient and flexible in your approach.
* Carrying out of assessments of all new packages.
* Meet and greet with our potential private clients and/or their families.
* Re-assess existing packages, for example, when a client is discharged from hospital with a change in their needs, etc.
* Carrying out required risk assessments to ensure delivery of safe care.
* Updating care plans, tasks, risk assessments, medication information and other care related documents found within the client's electronic care plan and care folder, as appropriate.
* Undertaking client reviews under the direction of the Care Coordinator and Deputy Manager.
* Undertaking of staff observations and/or supervisions under the direction of the Deputy Manager/Manager.
* Liaise with other health and social care professionals.
* Undertake care work, when required to meet the needs of the business.
* Working hours to include a minimum of one evening and an alternate weekend.
* Be part of the on-call rota, ready to cover any outstanding care visits at very short notice.
* Available to attend Team Leader Meetings on a monthly basis
Core Responsibilities:
1. To maintain your uniform and all company property issued to you to a standard compatible with Candlelight Care's expectations. Being clean and smart will add confidence and demonstrate your professionalism to others.
2. Refrain from wearing Jewellery, False nails or nail varnish whilst on duty, leading by example and promoting good infection control procedures.
3. You are required to conduct yourself at all times in a cordial, respectful and courteous manner, even in difficult situations.
4. Maintain good working relationships with your Office Team, other care and support workers, clients and their families/representatives.

The post-holder will be subject to an Enhanced Certificate from the Disclosure and Barring Service. We will undertake to discuss any matter(s) revealed on the Certificate with the person directly. Having a criminal record will not necessarily bar you from working with us. This will depend on the nature of the position and the circumstances and background of any offences.
Essential Criteria:
Available to work one evening and alternate weekends
Adaptable and flexible to deal with varying needs and situations
Be part of the on-call rota
Willingness to cover visits at very short notice when covering the On-Call
Previous experience within a care role.
Other caring roles (e.g. caring for family members)
Good working knowledge of care and support work.
Understanding and respecting the need for confidentiality
Good working knowledge of Health & Safety
Good working knowledge of personal rights and responsibilities
Awareness and importance of preserving clients dignity
High level of literacy skills (you will be required to read care plans and record simple details)
Computer literate
Diploma level 2 in Health & Social Care
Good communication skills
Ability to work without constant supervision
Caring and empathetic approach
Reliable and trustworthy
Clean and tidy appearance
Ability to manage your own time
Good team player, who is understanding and supportive of colleagues
Good communication skills (to understand clients and make yourself clearly understood?
