Youwill need to be friendly and approachable with effective communication skills,able to build good working relationships within a fast-paced environment. Aswell as being a team player, you will need to demonstrate sensitivity andconfidentiality due to the nature of our services, work autonomously andpromote a flexible approach and strong work ethic to prioritise, multi-task,and delegate.
Weare looking for an organised, reliable administrator to join the Care Hometeam. They will support the wider team with general admin tasks and have a highlevel of organisation and the ability to adapt to new tasks as and when theyarise.
Main duties of the job
KeyResponsibilities:
Managing the Care Home teamgeneral inbox
Answering and actioningrequests that come through the teams phone line
Data entry and updates ontoExcel Spreadsheets and patient records
Generating reports for theClinical Team
Formulating templates toassist the team
Creating how-to guides forcomplex templates used by the team
Supporting the team with ITrequests
Spreadsheet management
Liaising with care homes andother services
Send referrals to otherservices on behalf of residents of care homes
Assisting members of the teamwith home rounds; notetaking, typing up, updating systems
Take meeting minutes fromweekly MDT and ad hoc meetings
Provide admin support to helpthe clinical team manage residents who are admitted discharged from hospitals
Manage blood requests forresidents of Care Homes
Support with coordination ofChronic Disease Management reviews
Input data on Personalisedcare and support plan templates
Support team in generatingEPaCCS on EMIS
Other admin tasks scanning,printing, uploading documentation
Monthly tasks pullpopulation reports, gather data
And any other duties which areappropriate to the role and support the wider team

Primary Care Networks
A Primary Care Network (PCN) enables the provision of proactive, accessible, coordinated and integrated primary and community care, improving outcomes for patients. They are formed around natural communities based on GP registered lists, serving populations of around 30,000 to 50,000.
The Lytham St Annes Primary Care Network includes, 5 GP Practices, Holland House, Fernbank, Parcliffe, Poplar and Ansdell, serving a population of 53,772 registered patients.

Skills and Attributes
* Evidence of excellent knowledge of Microsoft Office
* Able to work as part of a team
* Able to prioritise and manage your own workload
* Excellent organisational and administration skills
Experience
* Good level of communication skills both verbally and written
* Good organisational skills and time management.
* Professional, calm and efficient manner.
* Self-motivated, can work well independently and within a team.
* Previous primary care experience either in GP practice or Community Teams
* Experience of NHS IT Systems EMIS

Qualifications
* Administration experience within the NHS
* Microsoft products including excel teams outlook etc
* Experience of diary management
* Good General Education (eg. GCSE English and Maths A-C)
