NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.
The role:
The Long Term Conditions AHP Team provide an essential and specialised therapy service for all patients referred to the team diagnosed with long covid and/or other multi-morbidities. Including conditions that have been impacted or exacerbated by the covid19 pandemic.
The team will work remotely with bases in Gorbals Health Centre, Rowan Business Park and Stobhill Hospital providing a service to all general practices within the six HSCPs within NHSGGC boundaries.
The department provides:
* highly specialist care and advice
* support and expert advice including Supported Self-Management techniques
* Signposting to community resources and digital solutions
* Quality out-comes for the patients referred to these services
All staff within the Services must comply with NHSGGC Policies in addition to this qualified staff will comply with Chartered Society of Physiotherapy (CSP) or Royal College of Occupational Therapists (RCOT) Professional Standards and Code of Conduct and HCPC Standards of Proficiency.
Health Care Support Workers are required to comply with the NHSGGC Mandatory Induction Standards and Code of Conduct
Duration, Location, and Working Pattern:
Fixed Term, 12 Months - Full time
Base location - Hybrid/ touchdown space at Rowan Business Park
Key responsibilities:
Direct Patient Care & Rehabilitation
* Works with patients individually in their homes, delivering delegated treatment and rehabilitation programmes.
* Adjusts treatment plans within agreed parameters and reports any unexpected changes in progress.
* Supports patients with mobility, including walking practice, stairs, outdoor mobility, and managing falls or faints safely.
* Assists with social and community reintegration, accompanying patients to activities and public transport.
* Performs manual handling, passive limb movements, and uses mobility/therapy equipment safely and competently.
* Implements therapeutic activities such as memory strategies, relaxation, anxiety management, fatigue management, and other self‑management techniques.
Clinical Monitoring & Interventions
* Carries out delegated clinical tasks such as monitoring blood pressure, weight, height, and other routine measures.
* Uses interdisciplinary assessment tools (e.g., Tinetti, Nottingham, Waterlow, MOHO).
* Coordinates interdisciplinary interventions from assessment through to discharge under supervision.
* Provides health promotion information within a set framework.
Communication & Team Collaboration
* Liaises with multidisciplinary team members and attends team meetings to provide updates on patient progress.
* Completes referrals to other services and maintains communication with external agencies.
* Provides advice and training to patients and carers on lifestyle changes, environmental adaptations, and the impact of disability.
Documentation, Data & Governance
* Maintains accurate patient records, documentation, and statistical data (including electronic systems).
* Collates and presents outcome‑measure data and supports audits, quality improvement, and clinical governance activities.
* Ensures confidentiality in line with Data Protection legislation and organisational policies.
Equipment, Stock & Administrative Support
* Manages stock levels, orders supplies, and completes associated administrative tasks.
* Uses clinical equipment requiring dexterity and safe handling.
Training, Supervision & Service Development
* Participates in supervision, e‑KSF, and personal development planning.
* Supports induction and training of new staff, students, and healthcare support workers.
* Contributes to teaching sessions, in‑service training, and structured information sessions for clients.
* Mentors students on placement and helps develop frameworks for monitoring care standards.
Safety & Professional Standards
* Adheres to Health & Safety policies to maintain a safe environment for patients and staff.
* Actively participates in service development and supports the efficient running of the Long‑Term Conditions Service.
Knowledge, training, qualifications and/or experience required to do the job:
Essential Criteria
* Experience working within a Health and/or Social Care setting.
* Full, current driving licence.
* Effective verbal and written communication skills.
* Working knowledge of basic IT systems.
Desirable Criteria
* SVQ Level 3 (or equivalent) or a higher qualification relevant to clinical work.
* Experience working with adults and children in a multidisciplinary, community‑based service.
* Training in mobility assessment, mobility progression, exercise classes, passive movements, and individual exercise programmes.
* Ability to work independently.
