The post holder will work as a member of the multi-professional team providing high-quality healthcare support, assisting in medication reviews and completing physical health checks with clients including ECGs, Blood Pressure readings and phlebotomy.
You will be required to travel independently to client's homes across Rotherham.
The Trust offers flexible working arrangements, clinical supervision, development planning and career planning. Due to the nature of this post, provisionally selected applicants will be asked to apply for a Disclosure and Barring Service check from the Criminal Records Bureau.
* Undertake agreed activities as delegated by a registered nurse, with patients/service users under the direct/in-direct supervision of qualified staff.
* Contribute to the implementation and delivery of individualised evidence-based care.
* Maintain accurate records electronically, record new problems as they are identified and report all actions to nurse in charge.
* Takes personal responsibility to incorporate the 6 C vision of care into their care delivery.
* Provide feedback of current patient/service user status to appropriate members of the professional team.
* Develop rapport with patients/service users and encourage them to participate in their plan of care.
* Work in partnership with patients/carers and multi-agency teams to promote health and independence.
* Assist the relevant professional during assessment/treatment/intervention where required.
* Support the patient/service user to make use of available services and facilities.
* Organise and run testing events in partnership with other agencies.
* Obtain follow-up bloods from patients with poor venous access.
* Follow health and safety regulations, promote a safe environment where possible.

The Mental Health Services for Older People offers inpatient and community services to people in the Rotherham area, who present with a range of mental health difficulties. We also offer specialist services such as a Young Onset Dementia Service, Memory Service, Care Home Liaison and Hospital Liaison Service.
