Overview
Clinical Team Leader - Hospital@Home role at Royal Free London NHS Foundation Trust. This is a 12-month fixed-term maternity leave cover, offered at 15 hours per week as part of a job share. The successful candidate will work 2 shifts per week as rostered with the line manager.
Responsibilities
* Lead the Paediatric Hospital@Home service and manage day-to-day operational delivery.
* Develop and implement bespoke care pathways to provide hospital-standard nursing care in the community and to avoid extended lengths of stay and unnecessary hospital admissions.
* Work within a virtual ward model, developing pathways and ensuring safe transfer home in collaboration with Paediatricians and pharmacy colleagues.
* Collaborate with multidisciplinary teams to ensure high-quality nursing care according to treatment plans and patient safety.
* Identify patients suitable for Hospital@Home, support patient flow and rapid discharges from ED or the wards; plan, implement and evaluate patient care; identify opportunities to develop management skills.
Requirements / Qualifications
* Extensive paediatric nursing experience with the ability to identify patients suitable for Hospital@Home and support patient flow and rapid discharges.
* Ability to plan, implement and evaluate patient care; some management experience or willingness to learn and develop management skills.
Hours
15 hours per week, part of a job-share, with 2 shifts per week as rostered.
