Job Title: Social Worker - Front Door team - Stockport
Location: Stockport
Pay Rate: £30
Organisation: Liquid Personnel is currently recruiting for a Social Worker to join the Front Door Team based in Stockport.
Responsibilities
* Work with Health colleagues to reduce hospital attendance.
* Complete visits (including same-day response).
* Conduct Care Act and short-term assessments to promote independence.
* Provide Social Work expertise in a multi-disciplinary team.
* Maintain an office-based role requiring a clean driving license and transport.

Qualifications and Experience
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
