Job Summary
Join our team of GPs and ANPs in the New Models of Care programme for the Northamptonshire Rural East & South (NRES) Neighbourhood. We are recruiting up to 8 GP sessions to work at Denton Village Surgery on our Neighbourhood Project. The role offers the chance to shape integrated neighbourhood services and provide high‑quality, same‑day care to our 88,938 registered patients.
Key Responsibilities
* Provide autonomous clinical care for patients presenting with undifferentiated primary care conditions in same‑day settings.
* Conduct telephone triage and clinic appointments to reduce demand on core GP practices.
* Use advanced skills to assess physical and psychosocial needs, initiate evidence‑based treatments, and review outcomes.
* Promote patient independence and self‑care where appropriate.
* Support and mentor junior colleagues and lead multidisciplinary teamwork.
* Maintain accurate clinical records, comply with professional and legal standards, and complete mandatory training.
* Undertake at least one clinical audit per year, evaluate data, present findings, and implement changes.
* Respond to complaints and clinical incidents, ensuring robust systems for joint learning.
* Involved in the delivery of the Quality and Outcomes Framework and NHS England/ICB/Federation Contracts.

Person Specification
Experience
* Great team working skills
* In‑depth knowledge base in the management of patients with chronic illness and minor illness
* Ability to maintain and monitor high standards of evidence‑based care
* Ability to direct and co‑ordinate care working autonomously and collaboratively
* Understanding of clinical governance issues
* Evidence of improving the patient’s experience
* Evidence of leading a service project at practice level (desirable)
* Undertaking research or involvement in clinical research programmes (desirable)
* Experience in writing clinical policies and protocols (desirable)
Qualifications
* CCT qualification
* On the performers list register for Northamptonshire
* Relevant first level degree
* Extensive clinical experience in primary care
* Evidence of continuing professional development
* Relevant Master level degree (desirable)
* Management qualification/experience (desirable)
* Post‑registration qualification in relevant areas (desirable)
* Teaching qualification (desirable)
* Recent attendance on a leadership skills development programme (desirable)
