PURPOSE OF POST:
Receive and co-ordinate referrals, undertake assessments/formulate care plans, arrange action and review services for children and families.
Experience
Some experience in social/health care field.
Skills/Abilities
Ability to make professional judgements in a multi-disciplinary setting.
Able to work with complex and challenging cases, where there may be issues of vulnerability and risk with appropriate supervision.
Education
Dip SW, CQSW, or recognised equivalent professional qualification
Registered with the Health and Care Professions Council
