Offer description

To provide on-the-day and pre-booked health care to patients. The role will involve the assessment, diagnosis, treatment, or referral of patients presenting with undifferentiated and undiagnosed conditions. The post holder will work within the Advanced/Emergency Care Practitioners level of competency.

The ECP will be required to respond to referrals and requests from various sources including:

* Care homes
* 111
* Out of Hours


Main duties of the job

* To provide assessment, diagnosis, and treatment at first point of contact by attending to patients according to their needs either in the Practice or patients' homes.
* To accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
* To evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral.
* To refer patients to appropriate care settings, within locally agreed healthcare pathways.


About us

Enfield Unity PCN comprises 9 practices across 16 sites. The post holder will be mainly based in 2 or 3 of these sites.

The successful applicants will have the opportunity to help shape the development of ECPs in general practice, working closely with clinical directors and senior colleagues. Job responsibilities


Clinical

* To provide assessment, diagnosis, and treatment at first point of contact by attending to patients according to their needs either in the Practice or patients' homes.
* To support and continue developing an on-the-day emergency patient service in the Practice.
* To accurately triage and prioritise patients, including in emergency situations, demonstrating the use of various techniques to elicit history, including past medical and drug history.
* To evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral.
* To undertake and interpret diagnostics and take appropriate action to manage patient care, distinguishing between abnormal and normal findings, relating them to possible diagnoses. To apply broad clinical guidelines (e.g., NICE), providing feedback on their interpretation.
* To signpost appropriately to other services within healthcare pathways.
* To refer patients to suitable care settings within locally agreed pathways.
* To discharge or treat as appropriate within scope of practice.
* To maintain accurate documentation of patient interactions and clinical care, including adverse events.
* To advise and support the public on healthy lifestyles, health promotion, and disease prevention.
* To provide clinical advice to team members when appropriate.


Communication

* To liaise with multi-professional teams across boundaries, including primary care, social services, and secondary care, based on patient needs.
* To maintain communication with healthcare professionals and outside agencies to ensure seamless, patient-centred service.
* To communicate effectively to ensure appropriate care and avoid unnecessary hospital admissions.
* To actively communicate with patients, carers, and relatives, meeting their individual needs.
* To handle complaints calmly and courteously, resolving them efficiently at a local level.
* To communicate effectively in challenging situations, including hostility.


Professional

* To use EMIS or similar clinical IT systems for data input and extraction.
* To collaborate with other healthcare team members, including district nursing, mental health services, care homes, ambulance, and secondary care.
* To ensure patient and staff wellbeing according to Practice policies.

Additional duties include participating in clinical audits, equipment testing, research, risk identification, and working within Data Protection legislation. Maintaining current registration and promoting confidence and trust are essential.


Person Specification


Qualifications

* Current registered Nurse (NMC) or Paramedic (HPC).
* Evidence of ongoing professional development.
* ENP/ECP Minor Injuries and Minor Illness Level 6 course.
* ALS/APLS provider.
* X-ray/IRMER course attended.
* Teaching and assessing qualification.
* Relevant MSc or BSc.
* Completed RCN Clinical Leadership Programme or similar.


Experience

* Ability to manage and prioritise workload.
* Proven ability to work autonomously and in teams.
* Experience with non-medical prescribing and PGDs.
* Experience as an autonomous practitioner in minor injury/illness settings or mobile services.
* Experience in ECP/Advanced Practitioner roles within primary or secondary care.
* Knowledge of government targets, NICE guidelines, and their clinical impact.
* Understanding of NMC/HPC Code of Conduct.
* Knowledge of child protection issues.
* Understanding of clinical governance.
* Basic IT skills and experience with patient documentation systems.
* Organisational skills.
* Ability to challenge professionally and appropriately, advocating for patients.
* Experience in triage/initial assessment, acting as shift leader if needed.
* Ability to work autonomously, using clinical judgment for diagnosis.
* Skills in managing minor injuries and dispensing protocols.
* Proficiency in multi-system assessment, ECG interpretation, and social assessments.
* Experience in emergency departments or similar settings.
* Experience in clinical audit and research projects.

Disclosure and Barring Service Check

This role requires a DBS check due to the nature of the work involving vulnerable populations.

Salary range: £53,000 to £58,000 DOE, including Inner London (HCAS).
