Job overview Join our Older Adult Community Mental Health Team as a NMC Registered Band 5 Community Mental Health Nurse based in South Staffordshire. As a band 5 Community Mental Health Nurse you will be part of a dedicated service that will provide specialist input to service users and their carers who may be affected by a mental health diagnosis. You will be responsible for the delivery of specialist assessments and evidence-based interventions associated to all areas of older adults’ mental health and dementia and contribute to delivering high-quality clinical services. You will hold a designated caseload, ensuring a comprehensive and tailored approach to care, engage in therapeutic interventions, following meticulous care plans, and making a positive impact on the lives of those navigating mental health-related difficulties. Flexibility is your forte as you navigate unpredictable work patterns and client bases, offering compassionate care in highly distressing situations. Assume the role of educator and advisor, making significant contributions to the supervision of junior staff. This role is for 18.75 hours per week and as part of this role you will be required to travel across a large geographical area. This role does not meet eligibility for sponsorship. Main duties of the job As Band 5 Community Mental Health Nurse, will be able to work autonomously on completing the assessments and formulating person centred care plans and risk assessments for those accessing the specialist service. As part of this role, you will be expected to: Contribute to the assessment, planning, implementation and evaluation of the evidence-based interventions required, for a named service user/group under the guidance of a senior practitioner. Promote service-users’ wellbeing and to assist them to maintain their place in the community. Be responsible for the appropriate clinical risk assessment of service users, providing an effective advice, guidance and decision-making process culminating in the coordination of any reviewed plan of care under the supervision of a senior practitioner. Be responsible to make clinical judgements backed up by clinical reasoning and recommendations regarding appropriate actions/sign posting in consultation with multi-disciplinary staff and in accordance with the clinical pathway and service specification.
