Permanent role with a Local Authority
Job Summary:
• We are seeking enthusiastic, self-motivated, and professionally confident Social Workers to join our Community Wellbeing (Adult Social Care) team on a permanent basis.

Key Duties/Accountabilities (Sample):
• Conduct Care Act Assessments and support planning using a strengths-based approach.
• Manage caseloads involving physical disabilities, learning disabilities, mental health needs, and older adults.
• Undertake Mental Capacity Assessments, Community DoLS, and Section 42 Safeguarding Enquiries.
• Participate in Continuing Healthcare (CHC) and Section 117 aftercare processes.
• Work collaboratively with other professionals, families, and communities to ensure person-centred outcomes.
• Maintain accurate and up-to-date records and contribute to case discussions and supervision.
Skills/Experience:
• Solid knowledge of adult social care legislation and statutory responsibilities.
• Proven experience of strengths-based, person-centred assessments and care planning.
• Competence in conducting safeguarding enquiries and mental capacity assessments.
• Effective communication and partnership working across multi-agency environments.
Additional Information:
• The role is full-time (37 hours per week) and based across locations in Hereford City, Ross-on-Wye, and Leominster (location preference can be specified).
• Qualified Social Worker with current registration with Social Work England
• Degree/Diploma in Social Work (e.g. DipSW, C.Q.S.W., C.S.S.).
• Interest or qualification in Practice Education, Best Interest Assessor, or Approved Mental Health Professional (AMHP).
