Offer description

Are you interested in joining a local authority that genuinely prioritises a supportive and people-focused working culture?

This Hospital Discharge Social Work position offers a flexible hybrid model, with the option to work from home up to two days per week. This helps you manage your workload alongside your personal commitments, while also reducing travel time and supporting a healthier, more balanced working week.

With adaptable working hours and a supportive team structure, this role is designed to help reduce pressure and improve overall wellbeing, particularly in a fast-paced hospital environment.

Some of the benefits include:

Local Government Pension Scheme (LGPS)
A secure, defined benefit pension that builds a reliable income for retirement based on your earnings and length of service, offering strong long-term financial stability.
Stable and secure employment within the public sector
Enjoy consistent employment with reduced risk of sudden role changes, providing reassurance and continuity in your career.
Enhanced annual leave entitlement (27–33 days plus bank holidays)
Additional time away from work to rest and reset, helping to maintain wellbeing in a busy hospital discharge setting.
Hybrid and flexible working options
A combination of hospital-based, community, and remote working to give you greater control over your schedule and workload management.
Clear development and progression opportunities
Structured career pathways that support progression into senior practitioner, specialist, or leadership roles within integrated health and social care services.
We are currently recruiting experienced Hospital Discharge Social Workers to join a dynamic and fast-paced multidisciplinary team. You will work closely with NHS colleagues, ward staff, therapists, and other professionals to support safe and timely discharges from hospital settings.

Your responsibilities will include undertaking Care Act assessments, coordinating discharge planning, managing complex cases, and ensuring that appropriate care and support packages are in place to promote safe recovery and independence in the community.

Applicants must hold a recognised Social Work qualification and be registered with Social Work England (SWE). Previous experience in hospital discharge, safeguarding work, and integrated health and social care settings is essential. Experience using Mosaic or similar case management systems is beneficial.

If this sounds like a role you’d be interested in, we’d be happy to hear from you.
