Engage with the Integrated Adult Community Care Team to provide expert consultation, support, and specialist advice on social care issues relating to adults. Act as an advisor within the multidisciplinary team, mentoring, educating, and supervising less experienced social workers and support workers. Conduct comprehensive assessments using standardised approaches, addressing the needs and concerns of service users and their families. Provide complex psychosocial support to service users and their families, advocating for them around emotional and practical matters. Lead the response to referrals, managing a dynamic caseload while respecting the dignity and independence of individuals. Develop and implement person-centred interventions in collaboration with the multi-professional team and community resources. Uphold Safeguarding Vulnerable Adults Policy, offering guidance and making necessary referrals to protect those at risk.
