Overview
We are seeking a dedicated and compassionate Hub Facilitator to join our team at Bassetlaw Day Opportunities. This 12-month post covers maternity leave. The successful candidate will ensure the provision of day opportunities for people and meet their care and support needs within our practice framework. You will work within a geographical area and be prepared to work at different buildings across the district.
Responsibilities
* Provide high-quality physical and emotional support and personal care to individuals, respecting privacy, dignity and specific needs.
* Develop care plans based on effective consultation with individuals, carers, specialist workers and other relevant agencies; monitor and review care plans and risk assessments.
* Work on a 1:1 basis or with groups of individuals at a base or in the community; deliver day opportunities activities in line with policy, legislation and practice standards.
* Provide cover and assistance in respect of staff absence and other requirements within a geographical area; use equipment as directed by the care plan and training received.
* Report health and safety issues relating to premises, vehicles, equipment and facilities; respond appropriately to crisis and emergency situations and escalate incidents as required.
* Collaborate with stakeholders, contribute to reviews, meetings and other forums; supervise staff as appropriate.
* Write, review and monitor care plans and risk assessments; undertake medication administration and recording procedures in line with policy.
* Support individuals to stay in control, developing and maintaining independence.
* Maintain relationships with individuals, carers and partners to ensure person-centred care and support.
Qualifications
* A Social Care qualification or equivalent, with a care qualification or working towards Level 3 Diploma in Adult Care (with a supervisory role).
* Knowledge of relevant legislation.
* Evidence of continuous mandatory training and development.
* Knowledge of main ICT systems in the service area.
* Excellent professionalism, efficiency, and effectiveness in service delivery.
