We are looking for a highly skilled, Band 6 NMC‑registered District Nurse to join our South Tandridge team. The role is part‑time, 22.5 hours per week, providing care across a 7‑day service and requires travel within the geographical area.
Main duties of the job
* Manage a caseload of patients as part of a team, supported by the Team Lead.
* Work with other health‑care professionals to deliver high‑standard care in patients' homes, aiming to avoid hospital admission and promote self‑care.
* Provide line management for staff and support the Team Lead with team development and well‑being.
* Drive a suitable vehicle, hold a valid driving licence and use it for business purposes.
Job responsibilities
* Deliver cost‑effective, high‑standard nursing care in the community.
* Continually manage a caseload of patients within a neighbourhood, focusing on hospital admission avoidance.
* Assess, plan, implement and evaluate individualised episodes of care to ensure safety and effectiveness.
* Collaborate closely with patients and carers, linking with acute hospitals and primary care, and maintain effective communication with all relevant agencies.
* Promote and develop evidence‑based practice in line with current policy and the NMC code of conduct.
* Line‑manage staff, overseeing their development and competence.
* Work shift patterns between 8 am and 6 pm, with the service operating 365 days a year.

* Good understanding of current nursing workforce issues.

Salary
£39,959 to £48,117 per year + 5 % HCAS per annum, pro rata.
