We are looking for a dynamic individual with Care Navigator experience to lead our team of experienced staff
We are a large GP practice based in West Derby. We are friendly, supportive and looking to train and develop someone with a passion for training and development of our Care Navigation Team
The role will include :
* Training and developing our team to support our patient population
* Management of the Care Navigation team on a daily basis
* Management of staff rotas and annual leave requests
* Working alongside other team leads to support the Head of Operations to meet local targets and contracts
Main duties of the job

* Complaints Handling
* Management of the weekly clinical team meetings and supporting clinicians with paperwork

Job responsibilities
RESPONSIBLE TO: Head of Operations
SURGERY: West Derby Medical Centre
HOURS: 37.5 hours per week
Purpose
The purpose of this role is to lead and manage the Care Navigation Team at the practice and ensure that all training to this team is monitored and developed as the role develops.
The post holder will line manage the Care Navigator team Performance. They will be responsible for activity management, and achievement of targets such as QOF, KPIs, and enhanced services alongside the Quality and Local Contracts Lead
This role also encompasses the responsibility of arranging and attending the monthly meetings held at the practice, ensuring all information is available to the Chair of these meetings
Responsibility for the management of Ad Hoc Clinics is also a key part of this role
General Responsibilities
* Establish and maintain effective mechanisms for all targets working alongside the Senior leadership team and administrative performance team.
* Keep abreast of new and emergent technologies to help drive the practice forward in terms of digital inclusion
* Work within the Senior leadership team to support with the practice strategy to be the best we can possibly be
* Be the accountable lead for attendance at monthly meetings to discuss complaints, Compliments, Safeguarding and GSF
Activity Management
To oversee and report to the Head of Operations, the practice daily workloads and challenges providing solutions to ensure targets are met
Work alongside the Quality and Local Contracts Lead to ensure all KPIs within QOF and GP Spec are being met
Data Quality
Develop and maintain templates and protocols within the electronic clinical system to maximise efficiency and support a consistent coding approach alongside the Quality and Local Contracts Lead
Induction and Welcoming Staff
Provide induction training for new starters, including GP trainees and support staff. Provide update and re-fresher training where required identifying this through the creation of a training plan
Other Duties
Act as an NHS Smartcard sponsor for the practice
Ensure all new staff have the necessary email / clinical session accounts to commence their role, also ensure that leavers are de registered
You will also be required to undertake any other duties that may be reasonably asked of you.
Person Specification Skills
A strong, motivated Leader
Experience
* Experience in a similar role at a Senior level
* Experience of Primary Care Working
* Experience with EMIS
* Experience with Paco Blinx
Qualifications
* GCSE A to C in English and Maths
* Good standard of education
* Experience within an NHS setting with EMIS / Clinical system experience

£14.50 per hour
Contract
Permanent to be worked between the hours of 08:00 20:00 Monday to Sunday
Depending on experience £15.00 per hour (flexible for good candidate)
