Community Support Worker

We have an exciting opportunity for a Community Support Worker to join the Dementia Assessment and Diagnosis Team on a 12‑month fixed‑term contract or as a secondment covering maternity leave. Secondments must be agreed by your line manager before you apply.
Main duties of the job
* Ensure the delivery of high‑quality care in the Dementia Assessment and Diagnosis Team with high personal standards.
* Provide care in line with the National Dementia Strategy, NICE guidance and relevant legal framework including the Mental Health Act 1983 and Mental Capacity Act 2005.
* Participate in procedures for prioritising work and caseload management under support and supervision.
* Participate in risk assessments and risk management plans to support positive risk taking where appropriate.
* Deliver non‑pharmacological interventions following specific assessments to address unmet needs.
* Provide signposting and support to people living with dementia via planned interventions in line with agreed care plans.
* Work flexibly across the countywide service based on skill and service need.
Person specification
* Qualification: Advanced Apprenticeship in (Mental) Health and Social Care or NVQ 3 in (Mental) Health and Social Care with evidence of numeracy and literacy skills (equivalent to GCSE/Functional Skills) or an equivalent health and social care qualification; evidence of continuing professional development and portfolio.
* Experience working within community mental health services specifically with patients with dementia.
Disclosure and Barring Service Check
Position requires a Disclosure and Barring Service check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
