We are now at the very start of an exciting journey of building a team of over 100 additional clinical roles to support and enable the delivery of services within each of our 4 PCNs. Main duties of the job
We are building teams within and across the PCNs to enable peer support and team working both within and across professions. We have a unique opportunity in Redbridge to create something special and we want you to be part of it. Purpose of the role: Working as part of Loxford PCN to support the Clinical Pharmacist team to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes. The role will also include training and development to support the wider MDT team improving co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.

The role will also include training and development to support thewider MDT team improving co-ordination and effective pharmacy administrationwithin general practice, especially with relation to repeat prescribing systemsand processes. MAIN DUTIES To undertake medicines reconciliation in accordancewith practice protocols and to update patients medical records / prescribingaccordingly. To support the achievement of the prescribing Quality and OutcomesFramework targets and other clinical targets.
2 Workingwith the Clinical Pharmacists and wider MDT team to contribute to the reviewand development of repeat prescribing protocols / Standard Operating Proceduresto improve the efficiency and effectiveness of repeat prescribing systems. Toprovide advice to GPs, staff and patients in changes to prescribing to supportthe improvement of prescribing safety, quality and cost effectiveness. Toparticipate in medication audits. To assist in the appropriate monitoring andmanagement of prescribing budgets and to liaise with the medicines managementteam at the Redbridge CCG. To prepare evidence based resources and informationto support the implementation of rational cost-effective prescribing. Tosupport the PCNs Clinical Pharmacists to provide medication reviews, patientinformation leaflets, medicine awareness; and to undertake community visits asappropriate. To take a proactive approach to the safe handling ofprescriptions, including assessment of risk and making recommendations forimprovement. To respond to medication queries from patients and staff in aprofessional manner referring to the appropriate GP in accordance with practiceprotocols. To advise on the sourcing and safe management of medicines asappropriate. To assist in the submission of claims for personally administereddrugs as required. Be responsible for the organisation, planning and of ownworkload to meet set deadlines. Responsibility for administration Takingmessages and passing on information as appropriate. Computer data entry/data allocationand collation, processing and recording information in accordance with practiceprocedures. Initiating contact with and responding to requests from patients,other team members and associated healthcare professionals and providers.Patient services To provide medication review services to patients via clinicsfor PCN practices, domiciliary visits and in residential and nursing homes, andto deliver pharmaceutical care plans that maximise cost-effective prescribingand improve the quality of patient care. Support the CCG prescribing supportpharmacists with the appropriate monitoring and management of their prescribingbudgets. To prepare evidence based resources and information to support themedicine management team and all other relevant health professionals in theimplementation of rational cost effective prescribing. To help plan, develop andsupport the introduction of new working processes within MMP to optimise thequality of prescribing.
