You will work in a team with other multi-disciplinary professionals including nurses, physiotherapists, occupational therapists, administrators and other support staff, on a rota system between the hours of 8am-8pm over a 7 day working period. You will work as a member of the team to support the organisation and facilitation of clinical care in a holistic and proactive manner to patients in our service.
Responsibilities
* Provide assessment and treatment to patients on the caseload with the guidance of the wider team
* Maintain a high professional standard of care in adherence to all protocols, policies and guidelines
* Manage a patient's care plan within your scope of practice and with the support of the wider team
* Provide clinical assistance to other members of the clinical team
* Maintain accurate electronic clinical records
* Work within our service that is delivered from 8am-8pm, 7 days a week
* Respond to referrals across urgent and planned pathways including hospital discharge facilitation
* Proactively monitor and manage patients on the caseload to ensure the best outcomes for them
Qualifications & Skills
* Excellent communication and clinical skills with experience of working within a team
* Proficiency in using electronic patient record systemsGood time management and ability to prioritise workload
* Many opportunities to develop clinical skills, physical assessment and clinical reasoning with the support of the wider team
* A full driving licence and access to a vehicle
