Role Overview
We are seeking to add an ANP Prescriber or Clinical Pharmacist Independent Prescriber to join our GP practice team to continue to deliver high quality primary care to our patients with long term conditions. The successful candidate will work closely with our experienced nursing, administrative and GP team to support high‑quality management of chronic diseases and ensure clinical governance in keeping with General Medical Services contract. This will include cardiovascular, respiratory, endocrine and metabolic diseases and involve both in person and telephone consulting. As a practice committed to teaching a wide range of trainees from medical students to senior GP registrars, the candidate would contribute and support the delivery of relevant education within the practice team.

What We Are Looking For
* Advanced Nurse Practitioner with prescribing qualification or a Clinical Pharmacist Independent Prescriber
* Experience working within primary care managing long term conditions is essential
* IT literacy in Vision and Docman or equivalent primary care system
* Ability to work autonomously with strong clinical decision‑making skills
* Attention to detail and commitment to delivering high‑quality chronic disease care
* Excellent communication skills and ability to work as part of a team
* Passion for improving patient outcomes alongside evidence‑based medicine
* Engagement in educational activities related to our status as a training practice
* Experience of clinical skills such as venepuncture desirable to reduce environmental impact of multiple attendances and aid continuity for our patients
Key Responsibilities
* Manage LTC reviews for patients on long‑term conditions registers alongside our experienced nursing team
* Make prescribing decisions in line with national guidance
* Optimise treatment plans and support medication adherence
* Liaise with the practice team and contribute to quality improvement
* Provide patient‑centred, compassionate care
* Deliver relevant LTC education for medical students, junior doctors and GP trainees
