We are looking for an enthusiastic, innovative, experienced Occupational Therapist to join our friendly, forward-thinking team providing rehabilitation for a diverse range of people with stroke and neurological conditions within Stockport.


Main duties of the job

Based in the community, you will work within a comprehensive multidisciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Clinical Psychologists, Nurses, Assistant Practitioner, Therapy Assistants and Admin. You will be welcomed as a valued member of the coordinated and committed team, sharing knowledge, skills and opportunities to learn.

You will be provide specialist knowledge to facilitate the early supported discharge of stroke patients from the stroke unit, and work with patients with acute and long term neurological conditions to provide rehabilitation in the home environment, and other community settings, improving their quality of life.

Support and education is provided with the opportunity to work directly with experienced and specialist clinicians across the team and the chance to contribute meaningfully to a fast-paced and growing team, who were awarded ' Clinical Team of the Year' 2023 award within the Trust .

Job responsibilities

You will undertake comprehensive assessments and use your advanced clinical reasoning skills to develop and deliver rehabilitation programmes that support patients to achieve their goals.

You will participate actively in our weekly MDT meeting, linking with therapists from other disciplines and helping to ensure fully co-ordinated communication and goal-setting. Within the multidisciplinary team you will provide support and training to our Band 5 Occupational Therapists, Assistant Practitioners and Therapy Assistants within the delivery of rehabilitation interventions and a supervisor, as well as providing clinical education to student members of the team.

As a member of the CNRS team you will have excellent assessment, communication and problem-solving skills and demonstrate an adaptable, flexible approach whilst meeting guidelines and standards.

Joining the Community Neuro Rehab Service (CNRS) in Stockport, you will see were a supportive, caring and proactive team. Everything we do is aimed at providing exceptional, holistic, person-centred care to people with stroke and neurological conditions within the community of Stockport.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.


Person Specification


Education & Qualifications

* Diploma/Degree in Occupational Therapy.
* State Registration with the Health Professions Council.
* Evidence of continual professional development.
* Fieldwork Education Course.
* Membership of RCOT
* Normal Movement Training


Knowledge

* Broad knowledge of current best practice in occupational therapy in physical and psychological health.
* Application of the Occupational Therapy process with designated patient group.
* Knowledge of occupational therapy models of practice.
* Knowledge and application of standardised assessments.
* Understanding of therapy outcome measures.
* Up to date in professional practice and new research.
* Knowledge of government policy and current practice.
* Knowledge of the principles of clinical governance since its application.
* Knowledge of intermediate care and community/support services.


Experience

* oPrevious experience as a Band 5 Occupational Therapist.
* oExperience in clinical areas covered by occupational therapy teams. Advice and support of junior staff, support staff and /or students.
* oPost registration experience in a stroke and/ or neuro-rehabilitation in a clinical setting.
* oExperience of research or audit.
* oExperience working in the acute hospital and community setting

£38,682 to £46,580 a yearpro rata per annum
