We are looking to appoint a Fracture Liaison Practitioner at the Ipswich Hospital, working mainly alongside the Rheumatology team and the community FLS team. You will preferably have existing knowledge and skills in orthopaedics, specifically osteoporosis care. Proven leadership experience at a Band 5 or above desirable but you will need to be able to demonstrate that they can work as part of the MDT but also autonomously. Main duties of the job To establish and develop skilled practice in the multidisciplinary management and nursing care of adult patients referred to the service with a fragility related fracture. Continuously prioritise workload and activity of patients. Maintenance of trust FLS database. Manage appointment lists including telephone and face to face clinics. Provide support to staff in directorate in awareness, assessment and treatment of osteoporosis and frailty fractures. To independently perform and be responsible for the various diagnostic assessments required for this patient group and within the Scope of Practice Manage their own list of referrals. Referrals to related services such as falls team, strength and balance service. Uploading of data to national databases. Work autonomously in planning, organising and managing own time and workload. To work within the framework of clinical governance to provide effective, high quality individualised nursing care to patients and relatives/carers, contributing to the continued development of nursing practice.
