Role Summary
Work as a full member of the primary health care general practitioner team, providing a full range of personal medical services to patients and contributing to the development of the practice.
Responsibilities
* Provide a full range of medical services as defined in the core GMS contract, additional and enhanced services where appropriate.
* Provide other personal medical services to meet identified patients' needs.
* Make professional, autonomous decisions in relation to presenting problems whether self‑referred or referred from other healthcare professionals within the organisation.
* Assess the healthcare needs of patients with undifferentiated and undiagnosed problems.
* Screen patients for disease risk factors and early signs of illness.
* Develop care plans for health in consultation with patients and in line with current practice; disease management protocols, provide counselling and health education.
* Work within current ICB policy for prescribing.
* Maximise the effective use of current clinical computer systems. Record clear and contemporaneous consultation notes to an agreed standard.
* Compile and issue computer‑generated acute and repeat prescriptions (avoiding handwritten prescriptions whenever possible).
* Take part in areas including health research, clinical auditing and data collection.
* Contribute to QOF requirements and recording data appropriately.
* Screen patients for health education.
* Attend team meetings and other clinical meetings as necessary.
* In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working with primary care, as set by the Royal College of General Practitioners, General Medical Council, other Royal Colleges and Councils applicable to the staff within the Practice, Department of Health, ICB, CQC, and all other regulatory and standard‑setting organisations.
Service Development
* Be involved in developing primary care services to meet the needs of the practice population.
* Work in close collaboration with the practice team to ensure that access targets are reached.
* Work with the practice team to ensure that immunisation and health inequalities targets as defined within the PMS contract are reached.
* Support health development and ensure health promotion work is undertaken to allow the practice population to make informed choices.
* Ensure the practice is represented at appropriate meetings, e.g. Practice Based Commissioning Groups.
Managerial
* Awareness of, and compliance with, all relevant organisation policies and procedures (e.g. prescribing, confidentiality, data protection, health and safety, infection control).
* Committed to life‑long learning and audit to ensure evidence‑based practice.
* Contribute to evaluated audit and clinical standards setting within the organisation.
* Contribute to the development of computer‑based records.
* Contribute to summarising of patient records and read‑coding patient data.

* Prescribe safe and effective medicines management which conforms to:
o NICE Guidance and DH directives
o Good Prescribing Practice as defined by BNF guidance
o Operate safe management and regular review for all patients receiving repeat prescriptions
o Controlled Drugs Legislation
o Shared protocols agreed between the ICB and other NHS providers
o Locally specified Patient Group Directions
o Work effectively so as not to exceed the Practice prescribing budget as set annually by the ICB.
o Comply with targets set within the Prescribing Incentive Scheme.
o Ensure referrals are made for patients to appropriate providers with sufficient information for assessment.
o Support complaints policy and procedure to ensure proper investigation and follow‑up action.

* All patients have the right to expect that all practice staff will respect their privacy and act appropriately at all times.
* In the performance of duties the post‑holder may have access to confidential information relating to patients, carers, staff and other healthcare workers. All such information is to be regarded as strictly confidential.
* Information may be divulged to authorised persons in accordance with the organisation's policies and procedures relating to confidentiality and protection of personal and sensitive data.

Person Specification – Qualifications
Essential
* Full GMC registration (MBChB or equivalent)
* UK permit or right to work in the United Kingdom
* GP vocational or additional experience in general practice
