Meadow House Hospice is excited to announce the opportunity to join our well-respected allied health professional team. This service offers the opportunity for staff to be at the frontline of developingand leading on the way we deliver care to our patients, their families and how we offer support to our partners in caring.We are looking for someone who is passionate about delivering exceptional palliative care. They must be skilled in collaborative working with a variety of healthcare professionals,able to manage complex patient care needs with compassion and efficiency and committed to improving patient outcomes and supporting our service expansion and development.
Main duties of the job
To provide a high quality and efficient physiotherapy service to patients with a wide range of physical and medical conditions referred to the Specialist Palliative Care Service.
To work as an integral member of the multidisciplinary team at Meadow House Hospice, assisting in the provision of support and rehabilitation to adults, in all End of Care Life settings including the Inpatient Unit, Day Services and the Community
To provide clinical expertise in the prioritisation, assessment, and treatment of patients within the scope of the conditions referred to the Specialist Palliative Care service. To ensure that service provision supports the objectives of End-of-Life Care provision across Hounslow Specialist Palliative Care ServiceWe are looking for someone who is:- Passionate about delivering exceptional palliative care.- Skilled in collaborative working with a variety of healthcare professionals.- Able to manage complex patient care needs with compassion and efficiency.- Committed to improving patient outcomes and supporting our service expansion and development.

Person Specification
Education and Qualifications
* oDegree in Physiotherapy
* oHealth Professionals Council Registration
* oSignificant post-qualification experience including relevant specialist areas i.e. palliative care, oncology, neurology or community
* oBe a member of a relevant Special Interest Group and/or CSP
* oEvidence of attendance at postgraduate courses relevant to specialist palliative care
* oEvidence of continuing CPD
Knowledge and Experience
* Experience of clinical audit, clinical governance and managing risk
* oExperience of managing an independent caseload where patients have life limiting conditions, and an understanding of specialist palliative care in practice
* oEvidence of staff supervision
* oAccuracy in patient assessment, treatment planning, goal setting and recording information in all settings.
* oAble to participate in end-of-life education
* oExcellent negotiating and leadership skills
* oAn understanding of providing services to a diverse population in the community
Skills, abilities and attributes
* oExcellent time management and organisational skills
* oGood problem-solving abilities
* oAbility to work independently and as a member of a multidisciplinary team in all settings of the service
* oCar driver/access to own transport
