We aim to influence the health of the north west London population beyond the 1.5 million patient contacts each year. We are seeking two passionate and engaging Population Health Senior Project Managers to deliver a set of objectives to improve population health and health equity, in partnership with Imperial Health Charity.
Main duties of the job
* To positively influence the health of north west London by embedding a population health approach across services and engaging communities outside hospital walls. The approach focuses on health equity, prevention and the Trust’s role as an anchor organisation.
* Join a developing population health team and coordinate and deliver population health projects and improvement initiatives in response to identified needs.
* Work closely with clinical, operational and community partners to design, implement and evaluate interventions that improve healthcare access, experience and outcomes for populations with the greatest health needs.
* One role will focus on delivering projects in collaboration with clinical services at the Trust; the other role will focus on engaging with local communities and working with voluntary and community sector organisations. There will be overlap, and both roles will collaborate on health equity and prevention projects.
* Two fixed-term (24-month) roles with at least 50% in-person work at main hospital sites. Flexible working is supported, but roles require at least 0.8 WTE.
