Community Specialist Practitioner – Lower Lea Valley

The Community Specialist Practitioner will work within the Integrated team of Community Nurses, Therapists, CPNs, and Social Workers. You will be responsible for ensuring all health and social care needs are met to improve health outcomes, functional status, and quality of life, thereby increasing patient choice and preventing unnecessary hospital admissions.
Key Responsibilities
* Coordinate and deliver comprehensive health and social care services within the community.
* Collaborate with team members to develop and implement care plans.
* Monitor patient progress and adjust interventions to optimise outcomes.
* Reduce unnecessary hospital admissions and shorten hospital stay for those admitted.
