Patient Pathway Co-ordinator MSK

Sherwood Forest Hospitals are seeking a Patient Pathway Coordinator (PPC) to strengthen our patient response to accessing Musculoskeletal (MSK) services. The MSK service is part of the MSK Alliance, MSK Together, and works in partnership with other health care providers. This post is located at Ashfield Health and Wellbeing Centre.
The MSK Hub provides a single point of access referral and triage service for all primary care referrals and manages end to end pathways for people with MSK conditions in Mid-Nottinghamshire. Clinical delivery occurs across 4 main sites (Ashfield Health Village, Mansfield Community Centre, Kings Mill and Newark Hospital) and within some leisure facilities, and the team consists of physiotherapists, consultants, advanced clinical practitioners, and pathway co-ordinators who are passionate and enthusiastic about ensuring patients get the best service to meet their needs. The role of the PPC is to ensure people are supported to access care and treatments across multiple partners to achieve their health and wellbeing goals.

Knowledge Requirements
* Computer literate, with experience of using a keyboard with a high level of accuracy
* Knowledge of NHS or service provider in general
* Thorough knowledge of healthcare administrative systems and processes
* Knowledge of databases, spreadsheets i.e. PAS, Excel or similar applications
* Experience of using a full range of IT/Patient data systems
* Confidentiality and Data Protection
* Health and Safety

* Sound office clerical background providing knowledge of relevant procedures

Qualifications
* Good general education with GCSE English at grade C or above., (or equivalent).
* RSA II (or equivalent)

* NVQ in Customer Care
* Institute of Health Records & Information Management Diploma (IHRIM)
* European Computer Driving Licenc
* Shorthand speeds of 80wpm
