Overview
We are seeking an individual to support the next stages in the development of the newly established CAMHS transitions service. This role will be a pilot, run for 12 months initially, but the hope is that it becomes permanent, so we can continue to support young people from our CAMHS inpatient settings to the next stage in their recovery.
As a team we adopt a holistic approach, focusing not only on mental health but also on physical, social, educational and motivational needs. By employing the "social prescribing" model we help young people engage with life through activities that promote community connections, sports, education, employment, housing and more. Your role will empower young people to build confidence, improve wellbeing and work towards their long‑term health goals.
Responsibilities
* Contribute as a key member of the CAMHS Transitions Team, providing high‑quality care to young people and developing, planning and implementing individualised treatment programmes based on each young person’s specific needs.
* Critically assess and evaluate the effectiveness of care programmes, working collaboratively with the multidisciplinary team to ensure optimal outcomes.
* Provide expert clinical knowledge and skills across both inpatient and community settings, delivering evidence‑based interventions and direct care.
* Manage caseloads autonomously, ensuring effective time management while providing care and support to young people in both inpatient and community settings.
* Monitor and assess the impact of prescribed medication, providing timely updates to the line manager and ensuring the wider team is informed of any changes, fostering effective communication across teams.
* Engage young people in a person‑centred approach to goal setting and treatment planning, ensuring their views and those of significant others are integral to the formulation of care plans.
* Actively contribute to the development and improvement of the transition service, supporting the implementation of service changes and contributing to new initiatives within the team.
* Participate in professional meetings, including multi‑agency risk planning meetings, contributing to multi‑disciplinary discussions to ensure high‑quality care is provided.
* Maintain accurate and up‑to‑date records of care programmes and communications, ensuring compliance with the electronic patient record system (Rio).
* Demonstrate sound knowledge of and adhere to the principles of the Mental Health Act 1983 in practice.
* Collaborate with the wider professional network, contributing to care planning and ensuring integrated support for young people.
* Prioritise safeguarding in all aspects of clinical practice, attend relevant safeguarding training, address concerns with safeguarding leads, and ensure safeguarding issues are appropriately recorded on the electronic patient record system.
