An exciting opportunity has arisen within the Discharge Team at Worthing. We require a professional and passionate Senior discharge co-ordinator. This is a Band 7 role who will support the Discharge Lead and Discharge Team .
If you are experienced in leading a team, complex discharge planning, improving patient flow and aware of the principles of the Discharge to Assess Model (D2A)., we would encourage you to apply.

Main duties of the job
1. To work without direct supervision, autonomously managing a caseload of patients.
2. Provide support and deputise for the Discharge Team Lead
3. To adopt the principles of a Discharge to Assess Model (D2A). Align the discharge pathways including Pathway 1 (home or normal place of residence), Pathway 2 (bed-based rehabilitation), and Pathway 3 (complex Assessment within a nursing care setting) to the patients assessed level of need.
4. Take a ‘Home First’ approach, providing patients with support at home. Wherever possible, patients should also be supported to return to their home for assessment.
5. Promote and work to the good practice guidelines of a D2A model, where going home is the default pathway (with alternative pathways for people who cannot go straight home).
6. To act as a role model to colleagues seeking to maintain the highest standards.
7. Ensure that high standards of health and social assessments and documentation are completed.
8. Ensure the patient’s, family, carers are central in decision making for discharge, including promoting and protecting the interests and dignity of patients.
9. To provide support to the Discharge Team Lead and support leadership and management of the discharge co-ordination team at Worthing.
10. Deputise for the Discharge Team Lead undertaking agreed designated responsibilities.

Detailed job description and main responsibilities
· To meet regularly with individual team members for clinical supervision, to discuss complex patient cases and assist with finding solutions to discharge planning issues.
· To oversee assessments by team members of patients with complex needs and the planning and implementation of individualised care.
· Recognise risk by use of risk assessments and assist with mitigation of such in collaboration with MDT.
· To provide professional leadership in discharge planning practice across the Trust by facilitating training, providing advice and direct support.
· To respond to complaints and disputes regarding the discharge process in a timely manner, including those relating to Safeguarding Alerts, to achieve optimum outcome for the patient and/or Trust.
