Senior Community Mental Health Practitioner
Within this role, you will work with an established and supportive MDT. We currently utilize a blended model of virtual and face-to-face working, offering flexibility. The ideal candidate will have knowledge, skills, and experience working across the spectrum of complexity and severity in adult mental health. You will work collaboratively and flexibly with others to undertake assessments, develop and share psychological formulations, and create treatment plans. Responsibilities include managing a caseload, assessing and planning treatment for patients, working with patients to plan care, and optimizing treatment plans. You will also feed back to the MDT and collaborate within the team to achieve the best outcomes for patients.
