Job Overview
We are seeking a dedicated and compassionate Care Navigator to join our friendly team at Tavyside Health Centre. The Care Navigator will play a pivotal role in assisting patients in navigating their healthcare journey, ensuring they receive the necessary support and resources. This position requires strong organisational skills, attention to detail, and the ability to communicate effectively with patients and colleagues.
Duties
* Assist patients in understanding their healthcare options and services available to them.
* Coordinate appointments and follow-up care, ensuring seamless communication for patients.
* Maintain accurate records of patient interactions and updates using data entry skills.
* Be the first point of contact for patients visiting the surgery.
* Demonstrate excellent phone etiquette when interacting with patients and stakeholders.
Experience
The ideal candidate will possess:
* Proven experience in an administrative or clerical role, showcasing strong organisational abilities.
* Proficiency in data entry with a keen eye for detail to ensure accuracy in patient records.
* Excellent communication skills, both written and verbal, with a focus on phone etiquette.
* A compassionate approach to patient care, demonstrating empathy and understanding in all interactions.
* Experience in a customer service role or working with the public.
