Dietitian - Primary Care (Full-time, Doncaster, In-person)
About the Role
We are seeking an experienced Dietitian to join a dynamic NHS Primary Care Network (PCN) in Doncaster South PCN.
You'll play a key role in supporting patients with diabetes, weight management, frailty and other primary-care nutrition needs, working closely with GPs and the wider multidisciplinary team (MDT).
Key Responsibilities
* Provide advanced, autonomous dietetic care – from assessment and diagnosis to treatment and discharge.
* Lead on management plans for diabetes, weight management and frailty, while supporting other nutrition-related conditions.
* Empower patients through behaviour change and self-management strategies.
* Collaborate with the MDT to integrate dietetic pathways and reduce GP workload.
* Contribute to clinical governance, audits, and service improvement.
* Offer education and guidance to colleagues on referral criteria and best practice.

* HCPC-registered dietitian with strong skills in clinical problem-solving and patient-centred care.
* Excellent communicator with the ability to build rapport across diverse communities and clinical teams.
* Comfortable working independently and in a fast-paced primary care environment.
* Confident with modern digital tools and clinical record systems.
