Dorset Healthcare is seeking a Community Rehabilitation Assistant to join our supportive Integrated Community Rehabilitation Team based in Purbeck. This is a 30-hour per week position, spread over 4 days, including weekends on a rota basis.
The multidisciplinary team works with adult patients in their own homes, Community Hospitals, or care homes to promote independence and enhance quality of life by supporting patients to remain at home or be discharged home. Being a car driver is essential for this role.
You will join a motivated, caring, inclusive, and compassionate team, working alongside Nurses and Therapists to set goals and help patients regain independence. Opportunities for further learning and development are available, including applications for Nursing, Physiotherapy, and Occupational Therapy Apprenticeships.
Responsibilities include working mainly under close supervision of clinicians, with occasional autonomous work. Previous experience in rehabilitation or care, community or ward-based, is desirable.

Key duties include implementing planned Intermediate Care/Rehab in various settings, maintaining evidence-based practice, participating in learning opportunities, managing workload, assisting with administrative tasks, and recording personal development.
This role requires an Enhanced DBS check, which will be covered by the Trust. Applicants subscribed to the DBS update service can present a valid certificate.
