This unique position offers you the opportunity to work as part of a highly skilled community workforce who are the cornerstone of safe patient care. You will work collaboratively with the multi-disciplinary team to visit patients in their own homes and care homes to avoid unnecessary hospital admissions. We offer extensive training and development pathways at every level, from band 3 to band 8, including a preceptorship program to support you in your role and ongoing training.
We are seeking a highly motivated and ambitious District Nurse to join the wider Community Nursing Team, which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates, and Healthcare Assistants.
You will undertake clinical skills such as venepuncture, wound care, catheterisation, and end-of-life care including syringe drivers.
You will manage a caseload of patients attached to 1 or 2 GP surgeries, with responsibilities including managing patients with long-term conditions and multiple pathologies. You will provide holistic assessments of health needs, relevant to nursing care, and offer health promotion. The role requires a District Nursing Qualification and experience in managing a team. You will act as a role model, teaching and supporting staff and students in the community.
Postholders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and a vehicle for business use are required, with reasonable adjustments made for disabled individuals in line with the Equality Act 2010.
