Main Purpose of Job
The main purpose of this full-time position will be to serve as a member of the Communicable Disease team of the Worcester County Health Department, specifically assisting clients infected with HIV to improve health outcomes. This is done by using case management services by providing medical and social support. Job duties include, but are not limited to assessment, individualized goal planning, linkage to resources, assisting with applications/referrals to resources, and assisting with access to appointments for needed services to improve health outcomes. In addition, this position will provide education for clients, the community and staff. Home visits and participation in multidisciplinary team meetings are required. The selected applicant must be willing and able to travel to all Worcester County Health Department sites.
MINIMUM QUALIFICATIONS
Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
Notes:
1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
2. Candidates may substitute Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.
