PCN Paramedic

Do you want to work with patients where you can really make a difference to those most in need? An exciting opportunity has arisen for an experienced and highly motivated Paramedic to join North Solihull PCN. With a population of over 39,000 patients, you will support 6 GP practices, all rated "Good" by CQC.
Main duties of the job
You will be a valued member of our multi-disciplinary team, which includes Clinical Pharmacists, Pharmacy Technicians, Paramedics, Physiotherapists, Health and Wellbeing Coaches, Social Prescribers, Care Coordinators, and Nurse Associates. You will collaborate with local providers from primary care, community services, social care, and the voluntary sector to offer personalized, coordinated health and social care to local patient populations.
The role requires visiting patients in their homes or care homes. Applicants must have a clean driving license and their own car.

Job responsibilities
Role Summary:
Assess, diagnose, treat, refer, or signpost patients attending the surgery with undifferentiated or undiagnosed conditions related to minor illnesses or injuries.
Coordinate telephone triage, taking clinical responsibility for patients requesting urgent appointments.
Provide a specialist Paramedic resource within the multidisciplinary team, working autonomously within practices to deliver holistic patient care, including assessment, diagnosis, and treatment.
Use advanced clinical skills to educate and empower patients in self-care and informed decision-making.
Perform other clinical and administrative duties as trained and deemed safe, such as assisting with medical reports, safeguarding, and clinic support.
Person Specification
Qualifications
* Paramedic Registration with the HCPC
* BSc in Paramedic Science or IHCD Paramedic
* At least five years post-registration experience
* Non-Medical Prescriber
* Qualifications in Long Term Conditions
* Membership of the College of Paramedics
