Overview
We are seeking a motivated and clinically confident Community Diabetes Nurse Specialist to join our community nursing service. The role sits within the home‑visiting team, supporting housebound patients with complex diabetes needs and providing expert clinical leadership to colleagues. You will act as a senior clinician, using advanced assessment skills, clinical judgement and specialist diabetes knowledge to guide care planning, adjust prescriptions and reduce unnecessary visits.
Key Responsibilities
* Conduct advanced clinical assessment and management for patients with complex diabetes in their own homes.
* Review and adjust diabetes treatment plans, offering expert advice to minimise avoidable hospital admissions.
* Triage referrals, coordinate caseloads and ensure staff work at the appropriate level of competency.
* Oversee quality of care delivered by the team, manage incidents and complaints, and maintain high standards of clinical governance.
* Support and develop colleagues, role‑model best practice and help maintain a confident workforce.
* Identify and lead quality improvement projects, present proposals to senior leaders, and manage budgets where required.
* Provide hands‑on clinical support and supervision frameworks for team members.
* Collaborate with Primary Care Networks and the wider multidisciplinary team to ensure joined‑up, patient‑centred care.
Qualifications and Experience
* Current employment within Kent and Medway NHS Trusts, Kent and Medway Integrated Care Board (ICB) or Medway Community Healthcare, or Student/Newly Qualified Nurses.
* Certification in Nursing and the Nursing and Midwifery (NMP) programme.
* Strong interest and experience in diabetes care at a Band 6/7 developmental pathway.
* Willingness to undertake postgraduate diabetes training if not already completed.
