Locum Adults Social Worker – Preparing for Adulthood (PFA) Rotherham Metropolitan Borough Council £33.12 per hour (via Oscar Morgan-Done at Service Care Solutions) 6 Months Fixed Term I am currently recruiting a dedicated and experienced Locum Adults Social Worker to join Rotherham MBC’s Preparing for Adulthood (PFA) Team. This is a fantastic opportunity for a skilled practitioner who is passionate about supporting young people as they transition into adulthood. You’ll be working within a supportive team environment to ensure smooth and person-centred transitions for young adults with complex needs. Key Responsibilities Undertake Care Act Assessments to establish eligibility and develop tailored support plans. Complete Focused Reviews and manage Direct Payments effectively and accurately. Apply the Mental Capacity Act (MCA) to assess capacity and make Best Interests decisions. Carry out and contribute to Continuing Healthcare Checklists collaborating with health partners as needed. Provide robust support to young people transitioning from children's to adult services including care leavers and those with SEND. Maintain high-quality, strengths-based casework ensuring legal and statutory duties are met.Requirements Qualified Social Worker with Social Work England registration. Demonstrable experience in adult social care with a focus on transitions/young adults. Strong knowledge of relevant legislation including the Care Act, MCA and CHC processes. Ability to work autonomously and manage a complex caseload.
