Enable Independence. Make Manchester Home. We are seeking an experienced and compassionate Qualified Adults Social Worker to join the Hospital Discharge to Assess (D2A) Team in Manchester. In this pivotal role, you will support individuals transitioning from hospital settings into the community, ensuring that care planning is timely, person-centred, and aligned with statutory responsibilities under the Care Act 2014. What You Will Do Undertake Care Act-compliant assessments and reviews for adults post-hospital discharge. Carry out Mental Capacity Assessments and act in Best Interests, where applicable. Support and facilitate discharges through D2A pathways, including community packages and intermediate care. Participate in crisis response services to prevent hospital admissions and deliver hospital-at-home interventions. Work one weekend in every four to ensure continuous service delivery. Develop and implement person-centred care plans focused on independence, safety, and wellbeing. Liaise effectively with NHS colleagues, partner agencies, and community teams. Provide mentorship and guidance to students, ASYEs, and junior staff within the team. Maintain accurate and timely case recordings in line with legislative and local authority requirements. Key Requirements Qualifications: Degree or equivalent qualification in Social Work Registered with Social Work England Experience & Skills: Experience in adult social care, hospital discharge, or D2A services Familiarity with the Care Act 2014, Mental Capacity Act 2005, and safeguarding protocols Strong assessment, planning, and case management skills Collaborative, confident, and effective in multi-agency working Desirable: Previous experience in a hospital or urgent care setting Knowledge of community resources and intermediate care pathways Familiarity with case management systems such as Liquidlogic or Mosaic Why Choose Manchester?
